Summary
The Suitcase Clinic provides primary care for homeless and low-income residents through the provision of volunteer nurse practitioners at a variety of local settings.  The VS worker is part of a community health team with a strong emphasis on data collection and case management fine-tuned to individual needs that may involve accompanying patients to medical appointments, working with particular patient needs and assisting with data collection and health care management.
