Support the Case Management Department by helping patients obtain public benefits, such as health insurance or Social Security benefits, and other social services, including mental health treatment and legal support.

Responsibilities include (but are not limited to):

  • Verify patient's insurance upon admission
  • Apply for health coverage for those who are uninsured
  • Help patients obtain identification
  • Accompany and serve as an advocate for patients on case management appointments at various agencies
  • Work closely/brainstorm with case managers in dealing with the issues that keep individuals homeless
  • Provide a daily presence to patients for their social work needs
  • Alternate Saturday coverage of the clinical unit front desk

*This position is best suited for an individual interested in a career in social work or other health related fields.

QUALIFICATIONS

  • Bachelor's degree required (a degree in Social Sciences preferred).
  • Experience in direct services preferred.

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED

  • Work experience with a difficult population preferred.
  • Good communication skills (written and oral), pleasant telephone manner, and the ability to use electronic medical records required.

SUPERVISORY CONTROLS
This position reports directly to the Lead Case Manager.

GUIDELINES
The position abides by all rules and regulations set forth by applicable licensing and regulatory bodies and the UHC policies and procedures.

PERSONAL CONTACTS
Primary contact is with the employees of UHC Health Centers and DOC sites, but may also include vendors, benefits providers, and local business establishments.

OTHER SIGNIFICANT FACTS
The incumbent must be able to balance the needs of diverse constituencies on a daily basis.

RISKS
The position involves everyday risk and discomforts, which require normal safety pre-cautions typical of such places as offices, meetings, training rooms, and other UHC health Care Sites. The work area is adequately lit, heated, and ventilated. The position requires contact with staff at all levels throughout the organization. There are also external organization relationships that may be part of the work of this individual. All medical services shall be provided according to medically accepted community standards of care. The employee shall provide evidence of recent (within the past twelve (12) months) health assessment that includes a PPD and/or chest x-ray results.
